FDT Monetary Pledge Form - USE THIS FORM TO MAKE A MONETARY DONATION TO THE Valley Community Coalition.

Use this form to make donations of TIME and/or funding to the VCC

In-Kind Log (PDF format)- Form to keep track of any and all time, donations, mileage   (money, supplies, food, etc) incurred by volunteers.  This is necessary for matching grant funds.  What does that mean?  Grant money is available to us if we match it.  For example, we can receive $100 in grant money if someone in the community donates another $100. As mentioned above, that doesn't have to be cash, but volunteer time/donations.  We strongly urge all volunteers to log their time/donations; this form is as important as the volunteer time itself!!

Valley Mentoring Program  Would you like to become a mentor?  Do you know a youth who would like to be part of our mentoring program?  This form allows you to fill out your contact information so our Mentoring Coordinator can make contact with you.  This page also provides more information on our mentoring program. 


VCC/SODA Pledge Form  (PDF) - Use this form to join the SODA youth group - open to Valley Community students grades 7-12

MENTOR/Mentee FORMS
If you would like to discuss the possibilities of being a mentor or locating a mentor for your child, please contact our Mentoring Coordinator,
Cassie Friederich at her email, cfriederich@helpingservices.org or at the VCC Office - 563-423-5171.  You can also opt to use the contact form at the bottom of this page.

The forms below are the same forms that we use to sign mentors up for the VCC Mentoring Program.  You may fill the forms out, or preview them to get a better idea of the required information. 
 
  • Mentor Application Form (PDF format) - this form is needed to officially apply to the VCC Mentoring Program.
     
  • Mentor Agreement (PDF format) - details the specifics of the Mentor/Student relationship as it pertains to the mentor.  Outlines specific behavior that is acceptable/not acceptable and protocol to follow in the mentor/student/parent relationship.  Signing this form indicates the mentor's agreement to the terms of the Mentor/Student relationship as outlined by the Valley Community Coalition (VCC).
     
  • Mentee Agreement Form (PDF format) - details the specifics of the Mentor/Student relationship as it pertains to the student.  Signing this form indicates the youth's agreement to the terms of the Mentor/Student relationship as outlined by the Valley Community Coalition (VCC).
     
  • Activities & Interests (PDF format) This page offers a checklist of activities/interests that the mentor/student can indicate that they are interested in.  With this information, we can more ideally match the mentor to a student with similar interests. 
     
  • Student Match Form (PDF format) - a form for students to fill out that best describes their interests, likes/dislikes and background information.
